Care Management

Care Management is a child welfare program that provides all elements of child welfare protective services to contracted entities and referral resources. We work in collaboration with the custodial county and the family to reduce risk, enhance safety and keep the family together in their home whenever possible. Care Management draws on the strong philosophy of employing the least restrictive,community-based services that incorporate Family Centered Practices and Family Group Decision-making Models of Practice.

Care Management Serves...

Families who are in the child welfare system due to circumstances of:

  • Abuse
  • Neglect
  • Status Offenses
  • Delinquency

Care Management Services Continuum

Through the Care Management Program, NYAP and/or contracted community providers offer:

  • Home Based Services
  • Residential Treatment
  • Community Support Services
  • Therapeutic and Family Foster Care
  • CANEI (Constant and Never Ending Improvement)
  • Alcohol and Other Drug Services

How is Care Management Different From Other Programs?

We give families a voice and a choice in their therapeutic care and support.
The program uses innovative yet realistic approaches to service delivery with measurable success and positive outcomes.

Contracted Care Management services are available throughout the Columbus Ohio area under contract with Franklin County Children Services.
